HealthChoices Patient-Centered Medical Home Learning Network
The HealthChoices Physical Health Managed Care Organizations (MCOs) are working with Pittsburgh Regional Health Initiative (PRHI) and the Health Federation of Philadelphia (HFP) to facilitate the HealthChoices Patient-Centered Medical Home (PCMH) Learning Network—one of the provider requirements for this PCMH Program. For the last eight years, PRHI has coordinated the PCMH Learning Network across Pennsylvania, facilitating quarterly, regional learning collaborative sessions for the Southwest/Northwest and Northeast regions with the HFP facilitating the Lehigh/Capital and Southeast quarterly, regional learning collaborative sessions.
The goals of the PCMH Learning Network are to support the PCMH providers and Physical Health MCOs in achieving the shared PCMH aims, identifying and acting on improvement strategies in response to opportunities for improvement toward the common PCMH aims and measures, and developing an internal capacity to continuously learn, adapt, and improve.
In 2025, the PCMH Learning Network will be replacing the regional sessions model with sessions organized by “Sprints” which are focused, concerted learning efforts to implement key interventions (manageable learning and implementation segments) within statewide groups of PCMHs working on the same focus area. The five Sprint focus areas include quality reporting and improvement strategies for blood pressure control and asthma medication ratio, child and adolescent well-care visits, including lead and developmental screening, social determinants of health interventions, post-hospital follow-up and preventing readmissions and the PCMH pediatric nursing care (PNC) program, with a Sprint focus on the PCMH-PNC Team and case managers.
